Please note: The following forms are for those who have undertaken the Part 1 Theory Course prior to January 2010.   

Those commencing training with the Theory Course from January 2010 will be required to complete a new application form and logbook which will be available to download from www.icgp.ie from beginning March 2010.

Form 1 - Standard Certificate in Family Planning

Form 2 - Certificate in Family Planning - IUCD Insertion (Optional)

Form 3 - Certificate of Insertion of Long Acting Contraceptive Implants (Optional) 

PLEASE NOTE THAT ALL THREE COMPONENTS FOR THE STANDARD CERTIFICATE MUST BE COMPLETED WITHIN 2 YEARS OF COMPLETING THE THEORY COURSE.
