Certification Fees are outlined as follows:

Form 1 - Standard Certificate in Family Planning                                           €100.00

Form 2 - Certificate in Family Planning - IUCD Insertion                               €50.00

Form 3 - Certificate of Insertion of Long Acting Contraceptive Implants     €50.00

Re-Issue of Certificate(s) -    €30.00 / certificate                                                          

All cheques are to be made payable to the JCFP.
